Welcome to the online incident/accident report form.

 

This form can be completed for any incident/accident involving players, managers, coaches, other club officials, referees or parents/spectators that has happened at any football match or a training session, all reports MUST be submitted within 24 hours of the incident occurring.

 

This form can be completed anonymously, but please be aware the club may not be able to action disciplinary sanctions without witness accounts.

 

Once submitted this form will go to our Welfare Team and actioned immediately.
